University Home Care is currently seeking a Home Care Intake Coordinator to join our team. This position plays a vital role in providing excellent customer service and coordinating home care services for our clients. Responsibilities:
Process incoming referrals and coordinate initial client assessments
Collect and verify all necessary client information
Ensure accurate and timely coding of client records
Coordinate with healthcare professionals to develop and revise client care plans
Schedule and coordinate home care services based on client needs and availability of caregivers
Answer phone inquiries from clients, caregivers, and healthcare professionals
Maintain electronic and paper documents in compliance with regulations and company policies
Requirements:
Prior experience in a healthcare or home care setting
Excellent communication and customer service skills
Strong organizational and documentation skills
Ability to work in a fast-paced environment
Proficient in Microsoft Office and relevant software applications
Knowledge of medical terminology and coding
