A health services network in New York City is actively seeking a Licensed Practical Nurse (LPN) to join one of its growing long-term care facilities in Brooklyn.About the Opportunity: Start Date: ASAP Assignment Length: 13-weeks Shift: Day or Evenings or Nights Schedule: Full-time (5 days a week) or Part-time (3 days a week); must be available to work every other weekend (Saturday & Sunday) Hours: 7:30am to 3:30pm or 3:30pm to 11:30pm or 11:30pm to 7:30am Setting: Long-Term Care Responsibilities: Provide bedside nursing care including taking and recording vital signs (BP, pulse, respiration, temperature) and monitoring intake/output Observe residents for changes in condition-physical, mental, behavioral-and report to the supervising RN or provider Administer prescribed medications (oral, injections, insulin), manage IVs, catheters (Foley), and routine basic wound care/dressing changes Perform treatments such as tracheostomy care and finger-stick blood glucose monitoring Change sterile dressings, assist with wound checks, and support the removal or maintenance of tubes/catheters, as delegated Coordinate multidisciplinary strategies to address resident issues, as part of the healthcare team Chart all nursing care, treatments, observations, and interventions in electronic medical records (e.g., PointClickCare) Provide emotional support and patient/family education regarding disease processes, medications, and self-care Perform other duties, as needed Qualifications: 6+ months of Long-Term Care experience Graduate of an accredited Practical Nursing program NYS Licensed Practical Nurse (LPN)
