Behavioral Health Specialist (PACE Model)
We are looking for a dedicated Behavioral Health Specialist to join our Social Work team in San Jose, CA. This full-time position centers around utilizing the PACE (Program of All-Inclusive Care for the Elderly) care model, representing and advocating for the behavioral health needs of each participant. This role is pivotal in managing a caseload of participants, ensuring comprehensive therapy needs and documentation, and integrating both individual and group therapies tailored to our participants' diverse needs.
Salary:
$99,309 - $119,171 per year
Essential Duties and Responsibilities: Conduct comprehensive behavioral health assessments to determine care needs, preferences, and goals of participants. Actively participate in interdisciplinary team meetings to develop specific goals and interventions. Deliver and document behavioral health interventions as outlined in participants' care plans including symptom management, counseling, and crisis intervention. Arrange necessary resources and services, assist with care transitions, and provide both individual and group counseling and care management. Facilitate various forms of therapies such as art, music therapy, psychosocial drama, counseling, and discussions aimed at restoring or expanding participants' social roles. Provide training on behavioral health symptoms and interventions to staff members. Minimum Position Qualifications:
Master's Degree in Behavioral Health (MSW) or Doctorate in Psychology (PhD) required. Licensed by CA Board of Behavioral Sciences as an LCSW or Licensed by the CA Board of Psychologists as a Psychologist. Minimum of five (5) years of behavioral health experience conducting therapy, completing behavioral health assessments, and managing care planning needs. Minimum of two (2) years of experience with a frail or elderly population. Preferred Qualifications:
Experience in interdisciplinary healthcare teams, particularly within the PACE program. Demonstrated ability to work autonomously and lead group therapy sessions. Strong interpersonal skills with a proven ability to communicate effectively (both written and verbal) with all levels of staff and management. Strong organizational and time-management skills. Skills:
