About the Spanish Speaking Case Manager Role As a Case Manager within our Long Term Services & Supports (LTSS) team, you'll play a vital role in supporting adults with physical disabilities to access the care and services they need to live with dignity-whether in a nursing facility or at home. This position is ideal for someone who is compassionate, organized, and comfortable navigating complex systems with empathy and clarity.
Starting Rate: $20.48/hour, per the current union agreement
Spanish Speaking Case Manager Core Responsibilities Deliver culturally responsive, trauma-informed, person-centered care Conduct comprehensive assessments and develop individualized service plans Link individuals to authorized services and community supports Monitor participant satisfaction, safety, and adherence to service plans Ensure Medicaid eligibility is maintained and assist with resolving related issues Maintain accurate and timely documentation Collaborate with internal and external stakeholders to coordinate care Provide crisis intervention and support when necessary What You Bring
Bachelor's degree in social work, psychology, or a related human services field OR Three years of experience in a social service or healthcare-related setting OR Current Registered Nurse (RN) Ability to speak and write in Spanish Strong interpersonal and critical thinking skills Comfort with electronic documentation and remote communication A valid driver's license and reliable transportation Who Thrives in This Role We've found that people who are:

Location & Schedule This position is based in Luzerne County, PA. Work is primarily community-based and includes travel to homes and nursing facilities. Office hours are Monday through Friday, 8:30 AM to 4:30 PM.
