About the Service Coordinator - Case Manager Role As a Service Coordinator within our Early Intervention team, you'll play a vital role in supporting children under the age of 3 who have developmental delays by coordinating access to needed services and supports. This position is ideal for someone who is compassionate, organized, and culturally responsive, and who thrives in fast-paced environments where collaboration matters.
Starting Rate: $20.00/hour, based on experience and qualifications
Service Coordinator - Case Manager Core Responsibilities Request and complete service authorizations in a timely manner Conduct Family Service Plan meetings and ensure compliance with OCDEL standards Coordinate services and make referrals to support each child's development Participate in Multi-Disciplinary Team meetings to promote collaborative planning Develop and update transition plans to maintain continuity of care Maintain positive and professional relationships with families, providers, and community partners Document all services and communication accurately and on time What You Bring
Bachelor's degree in Early Childhood Education, Psychology, or a related field and at least 1 year of experience in a human services role;OR an associate's degree or 60 college credits in Early Childhood Education and 3 years of relevant experience Strong customer service and time management skills Ability to navigate digital systems and prioritize multiple tasks A positive, solution-focused attitude and ability to build rapport A valid driver's license and reliable transportation Who Thrives in This Role We've found that people who are:
