The Case Manager will have a background in Chemical Dependency and Behavioral Health and a strong desire to work with patients who may have co-occurring disorders.

The Case Manager will provide case management services to ethnically diverse populations, including homeless residents, in a Residential Care Facility setting for JWCH. They will work as part of a multidisciplinary team that includes LCSWs, Case Managers, Primary Care Providers, Psychiatrists, and Counselors.

This position requires comprehensive case management, including the development of personalized care plans and coordination of various resources to enhance residents' well-being.

Applicants should have experience and a commitment to providing patient-centered care with unconditional positive regard and without judgment for individuals with complex medical and psychosocial histories. This includes people experiencing homelessness, those who use drugs, engage in sex work, and/or have been incarcerated.

The position also requires an understanding of, and ability to provide, care that adheres to principles of harm reduction and trauma-informed care. This includes, for example, recognizing unhealthy drug use as a complex phenomenon driven by structural violence, racism, poverty, trauma, and social isolation, and a commitment to providing care that acknowledges these forces.

Program Population:

Crocker Residential is a licensed Residential Care Facility for the Elderly (RCFE). We provide care and supervision for seniors aged 60 and older who require non-medical care and supervision in a home-like environment. We provide 24/7 assistance with activities of daily living (ADLs), including grooming, dressing, and mobility support-along with meal service, medication reminders, and general supervision. Our residents are adults living with chronic health conditions who cannot safely live independently and can benefit from personalized care plans that promote dignity, independence, and social engagement. We maintain a small facility of 48 residents to foster close staff-resident relationships and a strong sense of community. In keeping with our commitment to resident quality of life, we will accept a limited number of residents who wish to keep pets. Applicants should be aware that, on occasion, they may be working around friendly pets and should have no severe allergies to or significant fear of dogs.

Principal Responsibilities: Utilizes compassion and understanding of harm reduction to aid patients seeking treatment for opioid, alcohol and other substances use in an outpatient primary care setting. Provides extensive knowledge and education to patients, family and community members for different FDA approved medications designed to treat those with opioid or alcohol use disorder. Works directly with Medication Assisted Treatment (MAT) Providers, Behavioral Health Workers and other Care Team members to design and determine comprehensive care needs. Accepts referrals from clients in the RCFE setting and administers assessments outlined by DSM V to determine level of care, offer menu of options and link appropriate care services. Provides intensive case management and psychosocial support to those entering the MAT or general counseling program to ensure engagement, retention and better treatment outcomes. Performs and provides comprehensive case management that includes personalized care plan development, plan implementation and follow-up outside of substance abuse or related support, that will support clients in their general well-being. Connect residents with essential social and support services, arranging referrals for specialized care (such as ophthalmology, behavioral health, podiatry, nutrition transportation, housing and more). Navigates patients through treatment including reminders for appointments and encourages treatment adherence. Collaborates with outside agencies to offer referral options. Documents accurately and timely in medical record system. Maintains client confidentiality per HIPAA Participates in all staff, team and community meetings as required. Other duties as assigned. Requirements:

Preferred experience and knowledge of Medication Assisted Treatment and medications available to treat opioid or alcohol disorders. Preferred knowledge and training in harm reduction, trauma-informed treatment, and evidence-based treatments for substance use and behavioral health, including Matrix, Seeking Safety, CBT, Relapse Prevention, Motivational Interviewing, and the Nurturing Parenting Program. Must have valid California driver's license, proof of insurance and excellent driving record. Excellent written, oral, organizational and documentation skills. Two years' experience in a health care setting is preferred. Ability to work independently and proactively, taking initiative. Willing and able to work with diverse populations including low income, homeless, trauma-involved individuals.
