HRA
Ensures quality and safe delivery of health services to residents1. Implements plan of care formulated by physician and DRC2. Provides accurate and timely documentation of care given3. Assesses and provides resident and family/care giver information pertinent to diagnosis or plan of care4. Administers medication per physician orders.5.Documents properly on MAR.6. Observes for any adverse reaction and notifies physician accordingly.7. Assembles resident charts, resident records and checks that all necessary forms are complete and accurate8. Prepares incident/accident reports for residents as needed per policy. Notifies physician and family post incident/accident per policy and documents appropriately9. Maintains safe and healthy working environment and uses safe work methods and procedures in accordance with clinical standards10. Assists resident in learning appropriate self-care activities11. Organizes and monitors associates and material resources necessary for meeting the needs of the residents under his/her care.12. Recognizes emergencies and life-threatening situations and initiates appropriate action(s)13. Maintains confidentiality and resident rights regarding all resident personal information.14. Performs any other duties requested by supervisor.15. The ability to work in a safe and alert manner.16. The ability to take ownership for associate's safety and the safety of the residents17. Is prompt and able to perform the required duties of the position on a regular, predictable basis22. Becomes familiar and understands how to report a missing person using the Safe Return program or equivalent23. All associates are responsible for maintaining a safe and secure environment for all community residentsQualifications/Skills/Educational Requirements:1. Graduated from an accredited program for LPN/LVN and is currently registered in the state of the community2. Must obtain valid Continuing Education credits as required by the state of the community3. Training in Alzheimer and Dementia with focus on Validation Therapy4. Minimum of one (1) year nursing experience in a health care facility.5. Experience in a long term care community and/or geriatric nursing facility preferred6. Experience in emergency care and first-aid7. Able to communicate effectively with all levels of management, associates, residents, family members, and outside contacts.8. Experience with Microsoft Office and Outlook software preferred9. Able to make independent decisions10.
