Clinician II
who has experience working with adults with Intellectual Disabilities in an ICF/ ID Residential/long-term care setting with supervisory experience for our
West Neck Intermediate Care Facility.

As QIDP, the position is responsible for conducting a comprehensive functional assessment, identifying all specific developmental strengths and needs, behavioral management needs, and the need for services regardless of availability. Develop, in collaboration with the interdisciplinary team and using a person-centered approach, an IPP that identifies discrete, measurable, criterion-based objectives the individual is to achieve, with timetables for expected mastery, and specific specialized and generic strategies and techniques to be used. The position requires strong advocacy skills. Provide guidance and instruction to direct service staff regarding all aspects of the plan. Provide continuous monitoring of each client's progress, making modifications to the IPP whenever needed and reviewing the plan at least every 90 days. Ensure that services provided by each discipline on the individual's team are integrated within the home as well as the day program, if applicable. This position acts as the social work designee under the ICF/IID regulations. The position will carry stand by duty on a rotational basis.
Duties:
Assist in the development and implementation of policies and procedures regarding the operation of an ICF/IID facility. Work closely with House Managers and nursing staff to ensure that all services in areas, such as active treatment, behavioral and medical, are completed according to procedures and in compliance with State Licensure, Human Rights, and ICF/IID regulations. Coordinate and chair the inter-disciplinary team for each assigned resident; gather appropriate assessment information and develop goals and objectives that can be consistently implemented by both day and residential programs. Ensure that each individual's medical-protective equipment and adaptive devices are in working order. Participate in staff meetings to provide training on all client programs and revisions to plans; Provide training to all staff, residential and day program, within 30 days of development of individualized support plans, documented on IPP training form maintained in each resident's record. Adhere to all regulations pertaining to the development and review of individualized support plans for assigned individuals. Maintain chart compliance per agency standards, as measured by CQI chart reviews and Chart Sweeps. Complete all documents included in the annual packet by the due date for each assigned individual. Conduct meetings on a regular basis to keep staff informed on important issues, including but not limited to training on all resident programs. Monitor all services delivered to ICF/IID residents to ensure consistency of service, regardless of environment, and implement changes as needed. Assist supervisors to maintain a safe and healthy physical environment for clients and staff. Attend events for self-development that will increase and enhance professional growth to the mutual benefit of the residents, staff, and agency. Maintain communication with residents, advocates, guardians, and inter-disciplinary team members, and act as social work designee for the facility.
