Schedule: Full Time Shift: Days, 5 8-hour shifts with a rotating weekend schedule The community youll be caring for: AdventHealth Waterman - 1000 WATERMAN WAY, Tavares, 32778 The role youll contribute: The Social Work Care Manager intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs, funding sources, and qualify for community assistance from various programs and agencies. They also assist with transitions of care or discharge planning, provide crisis intervention, and coordinate discharge plans for high-risk patients. The role involves receiving referrals from interdisciplinary team members including physicians, RN Care Managers, staff nurses, and others. The Social Work Care Manager collaborates with patients, families, and the care team to ensure patient-centered care coordination across the continuum. They monitor resources, escalate clinical care issues, evaluate post-hospital needs, develop transition of care plans, and facilitate their implementation. The role aims to optimize patient flow, ensure safe and smooth transitions, improve patient satisfaction and safety, prevent readmissions, and manage length of stay. Daily communication with the interdisciplinary team during rounds is essential. The Social Work Care Manager also provides education on resource utilization, medical necessity, CMS Discharge Planning requirements, and care coordination. They are knowledgeable about post-hospital services such as Home Health, Infusion Services, DME, Palliative Care, Hospice, outpatient services, and community organizations. The role adheres to departmental policies, ensures quality care and compliance, and participates in customer service and relationship building. The value youll bring to the team: Psychosocial assessment and interventions Handling referrals for complex psychosocial needs Providing assessments and reports on child abuse/neglect, domestic violence, elder abuse, child protection, sexual assault, and human trafficking Consulting on decision-making capacity issues, guardianship policies, and foster care/adoptions Qualifications The expertise and experiences youll need to succeed: Master's in social work (MSW) Minimum three (3) years' experience in hospital/medical social work Preferred qualifications: Knowledge of state and federal guidelines related to care management
