Job DetailsJob LocationGloversville-NPCS - Gloversville, NYPosition TypeFull TimeSalary Range$27.00 - $28.00 HourlyTravel PercentageUp to 50% DescriptionAs a Licensed Practical Nurse in Foster Care, you will coordinate appropriate medical care, dental care, and mental health services nursing care/services to a youth and adolescent population with emotional, behavioral and physical needs. You will ensure that medical treatment services meet the requirements of the regulating agency. You will report to a Registered Nurse and you will work onsite.Responsibilities:Communicate with medical providers to ensure proper medical care in accordance with state regulations. Provide client and family education regarding medications and treatments.Work within the multidisciplinary team to ensure proper communication for medial needsTravel to foster homes to review and collect medication logs.Nursing documentation using EHR.Update discharge planning needs.See foster children in foster homes as needed.Provide information to be included on the medical portion of treatment plans for children in care.
