POSITION SUMMARY (Licensure Required)
The Clinical Director of Care Management is responsible for the daily management of program services; supervision of staff, and interns; ensuring high quality client service delivery; compliance with program grants, contracts, record keeping of complete client data; and preparing for audits. This includes clinical oversight of the Care Management and Enhanced Care Management (ECM) programs.
PRIMARY RESPONSIBILITIES Ensure clinical oversight and high-quality client service delivery, including periodic client check-ins over the phone and addressing client grievances. Enhance and maintain a sustainable ECM program. Prepare and oversee all necessary ECM components including client's files, documents, protocols and procedures, measured deliverables, and communication with health plan partners. Train and modify assessment protocols in accordance with best practices and health plans regulations. Review client case files for quality, timeliness, and accuracy of all documentation (such as the required assessments, forms, care plan, progress notes, closing letters, etc.) are complete, signed and closed in a timely manner. Ensure the required client data management systems (such as Kintone, GetCare, etc.) match hard copy client files; and files are maintained with current clinical standards. Monitor that progress notes match billable units of service. Oversee and monitor program compliance to Wise & Healthy Aging policies and procedures; maintain proper documentation of all program services delivered to participants. Ensure that service units and purchase of services are clinically supported. Serve as the initial client point of contact for referrals from various entities such as the City, County, and partnering healthcare organizations, etc. Serve as the agency liaison in City and County meetings that directly address client matters such as SM Task Force, and APS ICS correspondence, etc. Be proficient at utilizing internal client data management systems. Provide initial and ongoing training, supervision and problem-solving to care coordinators. Assign Care Management clients to care coordinators, and interns/volunteers Evaluate that clients are being referred to the appropriate service modalities and access to essential benefits. Provide one-on-one clinical supervision of care coordinators, and interns/ volunteers as needed. Facilitate monthly staff meetings to review cases and coordinate training with staff, and interns/volunteers. Provide direct client service when there are challenging situations that require the support of a supervisor. Provide timely performance feedback both formally (introductory reviews, performance improvement plans and annual performance evaluations) and informally to direct reports. Serve as back-up support, as needed, for the Vice President, Care Management. OTHER RESPONSIBILITIES
Oversight of MSW year 1 and year 2 student interns Provide clinical supervision for staff who are designated as Associate Social Workers (ASW) working towards licensure Provide reporting and analytics to the Vice President, highlighting progress to reaching set goals. Other duties as directed to accomplish program goals. POSITION QUALIFICATIONS
Education
Minimum education requirement: A master's degree in social work, psychology, public health, or a related field. Licensure is required. A licensed clinical social worker and licensed psychologist are preferred. LVN and RN will be considered. Experience
Experience supervising employees who conduct fieldwork. Demonstrate strong clinical and diagnostic skills with mastery in bio-psycho-social assessments, care plan, and care coordination (e.g. needs related to dental, medical, and mental health, substance abuse, ADL, IADL, housing/living situation). Respond professionally to clients, constituents, and the community at large. Prioritize and organize workload while responding promptly to clients and partnering entities or agency departments. Demonstrate experience performing direct service in social services or a related field. Prior management responsibilities are handling case management duties. Field work experience with ethnically and economically diverse populations. Bilingual language skills are a plus (e.g., Russian, Mandarin, Farsi, etc.) OTHER REQUIREMENTS
Ability to measure and assess the impact of services and support and participate in critical elements of the program's success. Strong clinical, interpersonal, verbal, and written communication skills. Ability to effectively communicate expectations to team members and demonstrate team building. Ability to multitask effectively and prioritize work efficiently with competing deadlines. Ability to demonstrate sound judgment and decision-making skills. Proficient knowledge of Microsoft Office software programs. Satisfactory criminal background check is required. Valid driver's license, auto insurance and satisfactory criminal background check required. PHYSICAL REQUIREMENTS
Ability to lift 25 pounds. Normal conditions for an office environment
