As a member of the Housing Supportive Services team, under the direct supervision of the Housing Supervisor, the Housing Case Manager will support clients enrolled in permanent supportive housing programs through LAHSA and Pomona Housing Authority. This program serves individuals who have experienced long-term homelessness, history of trauma, and have long-standing health and mental health issues. The Housing Case Manager will help clients build life skills, budgeting and other housing related needs so clients can remain safely housed.

KEY RESPONSIBILITIES:

Client Care :

Conducts a needs assessment that includes basic physical and social needs. Develops goals and objectives with client. Provides follow-up to ensure clients are moving toward goals as per service plan. Carries a caseload in the Housing Department Provides One-on- One Sessions, as needed, with caseload. Conducts/participates in house meetings to support clients and promote a safe and amicable environment in the units. Liaison between property manager and permanent housing clients, when needed. Supports caseload for yearly housing evaluations Encourages clients to be engaged in program and program demands. Lead staff for the department responsible for identifying resources such as employment, volunteer opportunities and additional social services in the community. Leads coordination of activities (arts in craft and community engagement). Supports and guides clients in placing work orders/passes and or request as needed. Helps develop transition plans, including assistance with housing, aftercare services, etc. Overall support for clients during shift (medication monitoring, collecting/distributing mail, making copies, answering phones, and any other program related needs). Provides community referrals based on clients' needs/request. Schedule necessary visits to community-based clients, when applicable. Agency Coordination

Supports and coordinates with client's external services. Responsible for coordinating case conferences with external and internal agencies associated with caseload. Community collaboration/ meeting with local agencies to identify resources for program. Informs external and internal agencies/programs about client's progress/lack of. Participates in staff meetings, team meetings and in-service trainings. Keeping track of program inventory/donations. Completing rounds in shelter units and shelter scattered sites, Other duties as assigned by program supervisor. Service Documentation/File Review

Utilize electronic record systems as per Prototypes and contract requirements. Document One-on-One sessions with assigned clients in a timely manner. Data entry in Welligent, HMIS and CoC systems Completes VI-SPDAT assessment and updates POC in HMIS as needed. Creates service plans as required by contract. Creates/guides/supports the creation of behavioral plans for clients with behavioral challenges. Documents communications with internal and external agencies to ensure consistency in services. Participates in program audit preparations Other duties as assigned by program supervisor. Community Relations

Makes appropriate referrals and follows up to assure delivery of services. Attends LAHSA/City of Pomona meetings and trainings, as needed. Attends house related coalitions/groups to support programs efforts. Other duties as assigned by program supervisor. EDUCATION AND KNOWLEDGE, SKILLS AND ABILITIES QUALIFICATIONS

Education Requirements

Bachelor's Degree preferred in related field Experience

A minimum of three years' experience in a supported housing environment. Experience working with substance abusers, chronically homeless and/or re-entry populations. Experience with running groups and/or providing SUD counseling. Experience with efficient and effective treatment and management of clients with co-occurring mental health and substance use disorders. Knowledge of and experience with providing trauma informed, Housing First and Harm Reduction services Strong computer skills, including Outlook, Excel, Word and PowerPoint Background Clearance

Culturally competent and able to work with a diverse population Strong proficiency with Microsoft Office applications, specifically Word Outlook and internet applications Experience working successfully with issues of substance abuse, mental health, criminal background, and other potential barriers to economic self sufficiency Experience and/or a general understanding of Housing First and Harm Reduction models Skills and Abilities
