Description Assess patients in either inpatient or outpatient scenarios and establish personalized treatment strategies Provide physical therapy interventions that emphasize mobility, strength, balance, and functionality Collaborate closely with nurses, physicians, case managers, and other rehabilitation professionals to enhance outcomes Educate patients and their families about therapeutic objectives, discharge processes, and home exercise plans Keep meticulous documentation of evaluations, treatments, and patient progression in compliance with regulatory and facility standards Engage in coordinated care through participation in team rounds and interdisciplinary discussions Education

Doctorate in Physical Therapy (DPT) or a relevant degree from an accredited institution Certifications

State licensure as a

Physical Therapist CPR certification is preferred Additional specializations (e.g., in neurology or geriatrics) are advantageous but not mandatory Skills
