Job Description
PURPOSE OF POSITION:
The Social Worker V will be a member of a fully integrated mental health team. The role of the Social Worker Vis to provide oversight and supervision of Social Workers in the unit. The Social Worker V also independently as well as in collaboration with transdisciplinary teams, identifies psychosocial issues and concerns of patients and their families and provides appropriate clinical services to meet their needs.
Summary Of Duties And Responsidilities
Supervise the work of Social Workers levels II's and III's in the behavioral health OPD, ACT program and medical clinic and assigns work to subordinates; provides professional expertise; establishes supervisory controls and ensures the work performed meets the agency's standards and time frame. Provide oversight and supervision of all admission and discharge planning. Provide consultation and technical assistance to subordinate staff and others by researching and analyzing information recommending corrective actions to be taken; conducts conferences as needed. Assess staff training needs; develops and revises lesson plans to meet training requirements; conducts training sessions and workshops. Serve as the clinical leader for the assigned area of responsibility and provide weekly individual and in-person supervision for all assigned mental health clinical staff; review diagnostic, treatment, and level of care decisions and documentation; ensure adherence to policies and procedures; and conduct clinical case conferences. Monitoring of the completion status of screening and psychosocial evaluations assigned to social workers, as well as individual safety plans.
Service Specific Addendum
Provide supervision to level II and III social workers. Provide therapeutic interventions drawing upon a wide range of treatment modalities. Serve as a consultant to multidisciplinary treatment team regarding psychological assessment, and performs psycho-educational evaluation. Participate as a member of the transdisciplinary team in formulating and executing overall treatment and after plans. Provide professional and appropriate interventions in crisis or emergency situations. Participate in transdisciplinary performance improvement projects. Perform related duties such as quality assurance activities, representation at administrative meetings, etc., as assigned by the Director of the Outpatient Division. Other tasks that may be assigned.
In addition to the tasks desc1ibcd above the following standards must be adhered to:
Compliance with all departmental, corporate, and hospital regulations, policies and procedures and conformity to established time and leave requirements. Appropriate professional conduct at all times towards patients, families, visitors and co-workers with courtesy, respect, sensitivity to socio-cultural and age-related issues and awareness of privacy and confidentiality requirements. Participation in regular supervision and awareness of need to seek guidance from appropriate personnel as necessary. Attendance at relevant seminars and in-service training. Maintenance of appropriate and timely documentation. Compliance with established dress guidelines and requirement to wear identification badge while on duty.
Minimum Qualifications
Valid license as an LCSW issued by the NYSED, and two (2) years of supervisory experience, as described above.
