$132,000.00/yr - $180,000.00/yr Responsible for delivering patient-centered, team-based care within the scope of practice, collaborating with physicians, nurses, and medical assistants. The role involves providing advanced physical and psychosocial assessments, diagnosis, management of health and illness needs, and direct patient care across all ages and genders, including acute and chronic conditions. The Nurse Practitioner or Physician Assistant will also support supervision of nursing staff alongside senior medical staff. Clinical Quality

Provide high-quality, patient-centered primary care. Utilize huddles, care gap reports, and team efforts to meet treatment and prevention goals. Educate patients about their conditions and treatments. Make appropriate referrals to specialists. Prescribe and dispense medications as needed. Manage medication requests, documentation, and patient communication efficiently. Collaborate with behavioral health, dental, optometry, and other teams for integrated care. Assist in program development, implementation, and evaluation. Participate in quality assurance and peer-review activities. Address investigations, grievances, and payer requests. Patient Experience

Deliver care aligned with the health center’s values, demonstrating empathy and effective communication. Ensure patient satisfaction, aiming to improve survey scores. Lead initiatives to increase patient survey participation. Work with management to address grievances and improve workflows. Access/Utilization

Provide primary care per policies and privileges. Cover urgent care schedules. Order labs and tests judiciously. Provide after-hours on-call coverage. Lead problem-solving for late arrivals and urgent cases. Work with management to optimize appointment access. Supervision of Clinical Staff/Leadership

Supervise nursing staff in patient care activities. Lead and promote positive team interactions. Support staff development. Participate in meetings and training. Attend professional development events. Documentation

Maintain clear, accurate documentation of visits and encounters. Code encounters appropriately. Close encounters per protocols. Provide medication coverage documentation. Minimum Qualifications

Master’s degree from an accredited Nurse Practitioner or Physician Assistant program. Current BLS certification. Current Registered Nurse License. Nurse Practitioner or Physician Assistant License. DEA License. Board certification for Nurse Practitioners or Physician Assistants. Preferred Qualifications

At least two years’ experience as a Nurse Practitioner, preferably in community health. Bilingual in English/Spanish is highly desirable. Seniority level
