- CCM Medical Assistant Role and Overview n
We are seeking a dedicated and experienced Medical Assistant to join our Chronic Care Management team, providing support to doctors and their patients. The successful candidate will play a vital role in delivering quality care to patients with chronic conditions, working closely with medical providers to ensure the well-being of our patients.

- Essential Job Functions n
Collaborate with physicians to create care plans for patients with chronic conditions
nAssist in patient education and self-management support
nCoordinate follow-up appointments and ensure patients adhere to treatment plans
nMonitor patient progress and communicate any concerns to the healthcare team
nDocument patient encounters and maintain accurate records in the electronic health record system
nConduct medication reconciliation and assist with prescription refills
nPerform basic patient assessments and vital sign measurements
nProvide empathetic and compassionate care to patients with chronic conditions

- Education and Experience n
Completion of a medical assistant program (not required if you have experience in the medical field.
nProficiency in medical terminology and EMR systems, specially e-clinical works
nExcellent communication and interpersonal skills
nTeam player and able to work with others to achieve department goals.
nAbility to multitask in a fast-paced clinical environment
nCompassionate and empathetic approach to patient care
nBilingual due to the high number of Spanish-speaking patients

- Compensation and Benefits n
The hourly salary ranges from $18.00 to $21.00, the hourly rate is commensurate with experience and certifications.
