TAY Program Coordinator
for the Futures First initiative. About Futures First
Futures First is a transformative program supporting
Transitional Age Youth (TAY)
ages 16–28 navigating homelessness, justice involvement, foster care, behavioral health challenges, and other systemic barriers. We walk with youth from crisis to stability—and beyond. What You'll Do:
· Support daily coordination and engagement of services for TAY participants · Conduct intakes, needs assessments, and support the development of care plans · Coordinate referrals and provide warm handoffs to internal and external partners · Maintain accurate documentation in alignment with Medi-Cal and CalAIM standards · Assist in program logistics, reporting, and outreach efforts What We're Looking For:
· 2+ years of experience working with TAY or similar populations · Familiarity with trauma-informed care, case management, and youth support systems · Strong communication, documentation, and coordination skills · Bachelor’s degree in Social Work, Psychology, or related field preferred; lived experience valued · Bilingual (Spanish, Khmer, Tagalog) is a plus Success in This Role Means:
· Youth are consistently engaged, supported, and retained in services · Referrals are followed through and care coordination is effective · Documentation is timely, accurate, and audit-ready · Positive collaboration across teams and with service partners · A strong presence and role-modeling of APF's True Believer culture Why APF?
