Family Medicine, Internal Medicine, and IM PEDS SUMMARY This position aims to provide high-quality patient care and ensure effective clinic operations. Patient care includes providing comprehensive medical services for members of family. Services include but are not limited to: preventive care, immunizations, primary care and treatment, health education, prescribing of medications, ordering and interpreting lab testing, physical examinations, STD screenings, vision/hearing, women’s health, family planning, prenatal services, nutrition. Provider may be used in various locations depending on the organization's needs. Clinic shifts include 1 evening a week and 1-2 Saturdays/month. This position may also include supervising physician assistants and/or nurse practitioners and training resident physicians or other health provider interns/externs.
REQUIRED EDUCATION, EXPERIENCE and COMPETENCIES Provide continuous, comprehensive primary care for infants, children, youth, and adults according to current standards of care. Take patient histories. Perform or request diagnostic tests. Make diagnoses and prescribe medication as necessary. Administer vaccinations and perform follow-up examinations. Educate patients in disease prevention and health maintenance. Prepare all medical records, clinical reports, and correspondence. Support quality initiatives. Work in collaboration with the management team and providers. Complete all Core Curriculum training(s) required for the position. Perform all other duties as assigned. Licensure/Certification: Active Illinois State Medical License, DEA certificate Board Certified or Board Eligible based on governing board guidelines Providers are responsible for maintaining individual certifications as required by job function or by law and provide verification and recertification when management requests. Experience: Experience should be aligned within scope of training. Familiarity with Electronic Health Records (EHR) is preferred. Specific Provider Competencies: Analytical: synthesizes complex or diverse information; collects and researches data if needed Problem Solving: Identifies and resolves problems promptly. Technical Skills: Assesses own strengths and weaknesses. Customer Service: Manages difficult or emotional patient/client situations; responds promptly to patient/client needs; actively works to improve customer/patient service and satisfaction based on feedback. Oral Communication: Responds well to questions, participates in meetings. Written Communication: Writes clearly and informatively. Ethics: Treats people with respect, upholds organizational mission and values
QUALIFICATIONS To perform this job successfully, an individual must be able to satisfactorily perform each essential duty. The requirements listed below are representative of the knowledge, skill, and/or ability required.
EDUCATION and/or EXPERIENCE Graduate of an accredited medical school with a degree of Doctor of Medicine (MD) or Doctor of Osteopathy (DO); completion of an Internship and an Accredited Residency Program required. COMPENSATION AND BENEFITS Medical/Dental/Vision $185,000-$230,000 Malpractice/Tail Coverage $2K CME Allowance with 5 Days Paid Time Off 3 Weeks Paid Vacation. Increases to 4 Weeks After Year 4 3 Paid Summer Days (1 for July, 1 for August, 1 for September) 1 Paid Wellness Day 5 Paid Sick Days 11 Paid Holidays Short and Long-Term Disability 403b Loan Forgiveness through NHSC and other state programs OTHER QUALIFICATIONS Current driver's license, Current auto insurance Current and unrestricted medical license in the state of Illinois Current and unrestricted Drug Enforcement Administration Certificat.e Complete all required Continuing Medical Education
