POSITION SUMMARY: The purpose of this position is to provide high quality patient care and ensure effective clinic operations. Patient care includes providing comprehensive medical services for patients between the ages of 0-21, within scope of training. Services include but are not limited to: preventive care, immunizations, primary care and treatment, health education, prescribing of medications, ordering and interpreting lab testing, physical examinations, STD screenings, vision/hearing, family planning, prenatal services, nutrition. Provider may be used in a variety of locations depending upon the need of the organization. Clinic shifts include 1 evening a week and 1-2 Saturdays/month. This position may also include supervising physician assistants and/or nurse practitioners and providing training to resident physicians or other health provider interns/externs.

ESSENTIAL DUTIES AND RESPONSIBILITIES: Provide continuous, comprehensive primary care for infants, children, youth ages 0-21 according to current standards of care. Take patient histories and document appropriately in the patient medical record Examines patients for symptoms or physical information Shows good insight and judgment with relation to patient care Perform or request diagnostic tests appropriately Analyzes reports and findings of tests and examination to diagnose condition of patient. Make diagnoses and prescribe

medication/treatments

as necessary. Administer vaccinations, if needed Educate patients in disease prevention and health maintenance. Prepare all medical records, clinical reports and correspondences. Support quality initiatives. Work in collaboration with management team and providers. Complete all Core Curriculum training(s) required for the position. Perform all other duties as assigned.

QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

REQURED EDUCATION, EXPERIENCE and COMPETENCIES: Education: Graduate of accredited medical school with degree of Doctor of Medicine (MD) or Doctor of Osteopathy (DO) Completion of an accredited Pediatric Residency Program Licensure/Certification: Active Illinois State Medical License, DEA certificate Board Certified or Board Eligible based on the governing board guidelines Providers are responsible for maintaining individual certifications as required by job function or by law and provide verification and recertification when requested by management. Experience: Experience should be aligned within scope of training. Familiar with Electronic Health Records (EHR) is preferred.
