Experience working with elderly populations is highly preferred. Certifications: Required: RN License, CPR Certification Required: Board Certificate Verification (specialty-specific) Required: CA Medical License Verification Preferred: Bilingual (English/Spanish) - good oral and written communication. •Responsibilities Provide primary care to elderly patients within the PACE (Program of All-Inclusive Care for the Elderly) model. Develop care plans in collaboration with the Case Management team. Diagnose, treat, and monitor patients, ensuring high-quality clinical outcomes. Supervise nurses, ensuring effective teamwork and patient care. Participate in the on-call panel and provide care during evenings/weekends as needed. Work with interdisciplinary teams to coordinate patient care and improve health outcomes.
