Futurecare Clinical Dietitian Position

The Clinical Dietitian follows strict guidelines as outlined with FutureCares diet manual and the clinical nutrition policies & procedures manual. Salary Range $85,000-$95,000 The salary mentioned above reflects the potential base pay range for this role. Bonuses or other incentives (if applicable) are offered separately. Offers will consider such factors as overall experience, job-related qualifications, location, certifications/training, etc. Responsibilities
Plan, develop, implement and monitor the nutrition plan of care for each resident Provide timely nutrition assessments, care planning, and minimum data set (MDS) assessments as required The nutrition plan of care is accurately and clearly communicated to the interdisciplinary team, resident and families via verbal discussions and written communication. Develop nutrition programs and systems are routinely monitored as part of the performance improvement program Determine patients at nutrition risk via nutrition screening Provide nutrition education to patients/families Provide nutrition counseling and education to patients Assure proper nutritional care for patients by conducting nutrition assessment Provide medical nutrition therapy (nutrition assessment, nutrition screening, counseling, education, modified diet formulation, nutrition intervention) for in-patients and out-patients Provide advanced medical nutrition therapy (nutrition assessment, nutrition screening, counseling, education, modified diet formulation, nutrition intervention, nutrition support) for in-patients Provide nutrition services and medical nutrition therapy integrated with the patient's medical needs Develop nutrition education materials/tools for patients Develop and implement medical nutrition care plans Perform comprehensive nutrition assessments and documents nutrition interventions in patient medical records Assess patients for nutritional status Providing nutrition assessments and care planning for patients Evaluate nutritional needs and develop a nutrition care plan Providing medical nutrition therapy (nutrition assessment, nutrition support, patient intervention and education) to meet the nutrition care needs Participate as a member of the Food Service Management team directly managing patient feeding Provide recommendations for enteral/parenteral nutrition needs and orders nutritional supplements to meet patient's nutritional needs Qualifications
