This role requires a dedicated professional who can provide exceptional care to adult and geriatric patients, focusing on diabetes management and related health concerns. Responsibilities: - Conduct patient assessments and develop individualized care plans.- Perform routine clinical tasks including vital signs monitoring, medication reconciliation, and venipuncture/phlebotomy.- Assist in special procedures such as thyroid biopsies and endocrine tests.- Maintain accurate patient records and provide documentation using the Community Hospital charting system.- Educate patients and their families on diabetes management and overall health strategies.- Collaborate with interdisciplinary teams to ensure comprehensive patient care.- Engage in follow-up phone calls and pharmacy communications to manage patient medications and refills. Qualifications: - Current Basic Life Support (BLS) certification required.- Minimum of 1 year of nursing experience, with a preference for previous charge experience.- Proficiency in the Community Hospital charting system is required.- Familiarity with internal medicine and urgent care practices is desirable.- Strong skills in patient assessment, communication, and interpersonal interactions. Patient Care Details: - Patient Age Groups: Adults, Geriatrics- Daily Clinic Visits: Approximately 125 patients- Floating within the hospital may be required. Compliance and Documentation: - Compliance document submission is required 5 days prior to start.- Candidates must be willing to undergo labs within 72 hours of signing a contract.- Two professional references from previous employers are necessary for submission. Additional Information: - A valid state physician or compact license is required at the time of submission.- Candidates should reside at least 50 miles away from the facility. Why Choose Us?
