Phaxis Physical Therapist Position
At Phaxis, we know that physical access and mobility underpin every students ability to engage and learn. As a school-based Physical Therapist (PT) serving districts near Hicksville, NY, youll design and deliver therapeutic interventions that help K12 students overcome physical barriers and achieve their educational goals. Key Responsibilities
Comprehensive Evaluations: Conduct mobility, balance, strength, and functional assessments to identify each students needs IEP Development & Implementation: Collaborate in IEP meetings to develop goal-driven PT objectives and carry out individualized plans of care Therapeutic Intervention: Provide one-on-one and small-group sessionsboth push-inand pull-outaddressing gross motor, postural, and functional skills Classroom & Environmental Consultation: Advise teachers and support staff on adapting classroom layouts, equipment use, and activity modifications to promote inclusion Progress Monitoring & Documentation: Track student gains with standardized tests and observational data;adjust interventions as needed and maintain compliance with district and state regulations Family & Community Liaison: Engage families and external providers to coordinate care continuity and home-program exercises Job Details
Position Type: Contract, school-based role (option to extend into the 20252026 school year based on mutual interest) Hours: Full-time, 2540+ hours per week aligned with district calendar and student schedules Pay Rate: $40$60+ per hour Caseload: K12 students with IEP-driven PT needs;assignments based on district priorities What You Bring
Credentials: Current New York State PT license (required);limited permits accepted under direct supervision if awaiting full licensure Education: Graduation from an accredited Physical Therapy program (entry-level DPT or equivalent) Experience: Prior school-based or pediatric PT experience preferred Skills: Strong collaborative communication, adaptability, and proficiency with assistive technology and adaptive equipment Authorization: Eligible to work in the U.S. Without sponsorship Phaxis Benefits
