Location: On-site, Santa Ana, CA Caseload: 25 Clients per CM Pay: $25-26/hr Start: ASAP Duration: Temp-to-hire
Day to day:Provide referrals and linkages to primary and behavioral health services and other supportive services.
Collaborate with referring organizations to coordinate guests care and ensure guests receive the services and support they need to achieve and maintain health, mental health, and housing stability.
Assist with housing applications, complete supportive and subsidized housing paperwork, survey rental market for affordable housing.
Advocate with prospective landlords and identify appropriate permanent housing options for guests.
Provide ongoing information, referrals, linkages, and advocacy for all other identified needs
Facilitate psychosocial rehabilitation groups for guests as required.

Pay Details:
$20.00 to $26.00 per hour
