We are seeking a driven and detail-oriented PHC Coordinator to join our team and support the scheduling, communication, and coordination efforts that keep our clients safe and well cared for.

Position Summary: The PHC Coordinator will oversee all aspects of Private Home Care service delivery, including caregiver scheduling, client communication, documentation, billing coordination, and service compliance. This role is critical in ensuring high-quality, consistent, and responsive care for our clients and a positive, organized workflow for our caregiving staff.

Key Responsibilities: Coordinate and maintain daily schedules for PHC caregivers, ensuring coverage for all shifts and client needs. Act as the primary point of contact for PHC clients and their families, addressing concerns, changes in condition, or scheduling updates. Match caregivers with clients based on skills, preferences, and availability. Collaborate with the nursing and clinical teams to ensure appropriate care coordination and follow-through. Monitor and ensure accurate documentation, timesheets, and service logs for compliance and billing. Track and report attendance, punctuality, and care quality metrics. Support onboarding of new PHC caregivers, including orientation and scheduling setup. Respond to emergencies and staffing issues promptly and professionally. Maintain up-to-date records and ensure adherence to state regulations and agency policies.

Qualifications: High school diploma or equivalent required Minimum 1 year of experience in home care, home health. Strong organizational and time-management skills. Excellent communication and interpersonal skills. Ability to multitask and manage urgent needs under pressure. Proficient in EMR and scheduling platforms Bilingual (Spanish/English) preferred but not required.
