Job Opportunity: Care Coordinator

Homewatch Caregiver's of Katy is looking to hire a Care Coordinator. We only accept professionals who are truly interested and have experience in home care. The Care Coordinator position is a dual role that combines both in-office coordination and field caregiving responsibilities. This role is designed for a dependable and compassionate professional who will caregiving shifts. Be available for start new clients from Monday to Friday. Be available for caregiving responsibilities Monday to Friday from 8am to 4pm, weekends as needed. Coordinate and adjust schedules to meet client care needs every week. Address and resolve scheduling conflicts promptly and professionally. Communicating between staff, patients, and family. Three years of experience minimum as a CNA or similar position. Developing patient goals and monitoring progress. Training staff and creating schedules. Maintaining patient confidentiality and quality care. Ability to work independently and stay composed in high-pressure situations. Exceptional organizational and communication skills. Maintain open communication with clients, families, and office staff. Flexibility to fill in as a caregiver when necessary and new start clients. Must have prior experience in all personal care and safely transferring clients with gait belt and Hoyer lift. Must have a reliable vehicle, proof of insurance, and valid driver's license. Note: as a Care Coordinator, you may or may not be called upon to cover shifts; however, when an open shift arises, you MUST be available to pick-up said shift at all times of the day between 8:00am to 9:00 pm.
