Manager of Case Management
Location: Riverside, CA Schedule: Full-Time | Monday-Friday, 8:00 AM - 5:00 PM (Rotating Weekends, Flexibility Required) Position Type: Exempt Relocation Assistance: Available on a case-by-case basis Position Overview
A large, high-volume acute care hospital is seeking a skilled Manager of Case Management to lead a multidisciplinary team focused on efficient patient throughput, safe discharge planning, and regulatory compliance. The hospital supports over 500 beds and provides services across multiple inpatient units. Key Responsibilities
Supervise daily activities of 15-18 RN Case Managers and Assistants Manage staffing, discharges, transitions of care, and throughput operations Conduct rounds and provide mentorship and coaching to staff Troubleshoot delays in care and escalate systemic barriers Coordinate with payers and post-acute care providers to support discharge planning Track case management performance metrics and ensure regulatory compliance Collaborate with the Director of Case Management to meet departmental goals Qualifications
Education:
BSN required; Master's degree preferred Licensure:
Active California RN license (or eligibility/pending license) Experience: Minimum 3-5 years of acute care case management or nursing leadership experience Strong understanding of InterQual, M&R criteria, and payer systems Familiarity with regulatory standards and discharge planning protocols Preferred Attributes
