We're looking for a dedicated and empathetic Care Navigator to support patients and families over the phone from our Santa Ana headquarters. While this is not a remote role, it is fully phone-based with no direct patient contact. You'll be the go-to guide for patients navigating healthcare

offering emotional support, access to resources, and seamless coordination of care. What You'll Do

Manage a consistent caseload of patients and caregivers Serve as a trusted resource and first point of contact by phone Offer emotional support and practical guidance with empathy and professionalism Help families connect with vital resources such as transportation, financial assistance, and community support Coordinate with healthcare providers to align and update care plans Schedule appointments, follow up on referrals, and track next steps Document all interactions with clarity and accuracy Monitor patient progress and respond to changes in needs De-escalate challenging conversations with patience and care Educate and empower patients and families to take an active role in their care journey Contribute to internal team learning and quality improvement projects Assist with onboarding and mentoring new team members What We're Looking For

Exceptional communication and phone etiquette Strong listening skills with a compassionate, calm demeanor Organized, self-motivated, and able to manage multiple priorities Team-oriented mindset with a proactive, problem-solving approach Experience in senior care, dementia support, or case management preferred Ability to remain composed in emotionally charged situations Tech-savvy and comfortable using digital tools and care platforms Bilingual (Spanish or Vietnamese) a plus, but not required Minimum Requirements

Must be 18+ and authorized to work in the U.S. High school diploma or equivalent required Must work onsite in Santa Ana, CA (this is not a remote position) Completion of Medical Assistant program is a plus Prior experience in healthcare, social work, or care coordination is strongly preferred Why Join Senior Doc?
