Unit Coordinator/Admissions, River View on The Appomattox

The Unit Coordinator/Admissions is responsible for providing clerical support to the clinical team and the admissions team as a part of a customer-focused, service-excellence-oriented team. The Unit Coordinator/Admissions is also responsible for providing assistance to the Director of Admissions for networking, advertising, developing targets for future referral sources for census development, as well as the admission process at the center and for assisting the Director of Social Services as needed. Demonstrates excellent customer service skills and relationship building with families and employees. Demonstrates understanding of the call bell system and the phone system. Demonstrates an understanding of how to handle a service concern from a patient/family/co-worker/or vendor. Demonstrates use of proper phone etiquette and effective communication. Demonstrates ability to communicate via secure communication. Demonstrates knowledge of and use and expectations for sending and receiving email notifications as appropriate and directed by DON and/or Administrator. Verbalizes understanding of his/her role in Huddles and Staff Meetings. Demonstrates ability to submit a maintenance request. Demonstrates ability to understand and process all matters in EHR. Verbalizes understanding of storage and inventory of medical and office supplies; assisting Supply Coordinator to maintain unit inventory. Verbalizes understanding of medical supply charge system. Demonstrates proper infection prevention and control procedures. Attends all required meetings and in services. Demonstrates basic knowledge of fire and safety program. Demonstrates ability to schedule appointments and transportation for patients. Demonstrates ability to operate the required equipment. Must complete all required Relias training before annual review. Understanding and implementation of standards, policies, and procedures for the admission process. Assist with and/or administer and process all daily information for the census. Work as a team player with the pre-admission review process. Collect information and funds in securing admissions and bed holds. Manage multiple assignments in highly pressured situations, meet tight deadlines, and schedules. Formulate professional relationships with referral sources and health care partners in the community. Conduct and develop telephone etiquette, tours, and interviews with backup staff, assist in on-site hospital visits as needed. Review hospital referral information and make decisions on admission based on center policy. Assist with the Director as applicable with maintaining census development by tracking status of occupancy and targeting census mix. Assist the Director with Admission/Services Barrier Surveys and report findings to the Director of Admissions and Administrator. Ability to coordinate admissions by researching inquiries and conducting center tours. Assist in collecting all data to be processed and finalized during the admission. Notify all staff of scheduled admissions, LOA, bed hold, and discharges, update computer system to reflect changes. Ability to design and implement an admission process which meets all the federal, state, and local guidelines, as well as company guidelines, including the completion of all required documentation, family and/or resident interviews, communication with referred agencies, Administrator, Director of Nursing and/or Assistant Director of Nursing regarding the feasibility of admissions and appropriate placement. Conduct tours of the facility with prospective residents and their families. Ability to assist in managing department budgets, ensuring availability of needed equipment and supplies. Attend weekly Department Head meetings assigned and/or in the absence of the Director and carry out all decisions and plans made. Assist in community outreach and planning of Health Talk presentations. Use of PCC CRM Module and referral systems. Performs other duties as assigned. Qualifications:
High school education and health care experience required Experience working in a long-term care setting preferred Demonstrated experience working in public relations and/or sales promotion Proficient in Microsoft Office, including Word, Excel, PowerPoint, and Adobe applications. Excellent interpersonal, verbal, and written communication skills. Must be able to work in a fast-paced environment. Ability to handle confidential information with great sensitivity. Ability to be flexible with scheduling to accommodate occasional evening, weekend, holiday and overtime hours as business needs facilitate. Must be detail-oriented, possess the ability to multi-task, meet deadlines, and be able to make effective decisions and work as part of a service excellence team. Knowledge of medical terminology preferred. Physical Requirements:
