RATES:Malpractice Must be included in rates.Orientation 225 - half the hourly/callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours Hourly 450 - Per hour not OT Holiday 675 - Must be 1.5x the hourly rate. Per holiday hour no OT Nightly Call 750 - Per night call to hold the pager to include 2.5 hours gratis Holiday Nightly Call - 1125 - Per night call to hold the pager to include 2.5 hours gratis On Call Daily 3715 or below - Per 24 hours to include 4 hours gratis24 Hour Holiday Call Rate - 5572.50 - Per 24 hours to include 4 hours gratisCall Back 450 Per hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.
