This specific position will not have a designated caseload but will provide housing and rental assistance for children with intellectual and/or developmental disabilities and their families. The purpose of case management is to access, coordinate, and assure the delivery of services and supports required by individuals with intellectual and/or developmental disabilities. Case Management will assist eligible individuals under their individualized plan in gaining access to needed medical, social, educational, and other services. This is a limited-duration position. Limited-duration positions can last up to two(2) years, but can end at any time. Must be fluent in Spanish (speak, read, and write). Language testing is required. As the Housing Case Manager Ld, other duties will include: Assist families of children eligible for Multnomah County Idd services with applying for housing assistance Assist families with filing out applicable housing forms, monitoring the process to ensure timely submissions to the housing funding team Report possible protective services issues and provide initial information to assist the investigator Maintain case records and document actions taken in accordance with federal, state, and local policy Develop, submit, and modify necessary documentation to implement/change/adjust payments and program benefits Secure the needed resources to stabilize the situation of the client with developmental disabilities Ensure that the client is eligible for services to which they are entitled and receives the support that adequately meets their needs Diversity and Inclusion: At Multnomah County, we don't just accept difference; we value it and support it to create a culture of dignity and respect for our employees.
