Hospice Home CNA, PRN
Transitions LifeCare is hiring immediately. As one of the community's industry leaders, we provide exceptional home health, hospice, and palliative care services throughout our 8-county territory. Transitions LifeCare's goal is to care for individuals and families with compassion, support, and expertise as they navigate the end of life or life changing illness. The schedule for this position is 3 shifts per month- either 7:00am-7:30pm or 7:00pm-7:30am. Why you should join Transitions: Shift Differentials for weekday nights and weekends A Successful CNA has the following: Caring and compassionate attitude with true concern for people Strong customer service focus Ability to work collaboratively as part of a team Ability to multitask Responsibilities: Provide high-quality and compassionate care to patients per their individualized care plan Works collaboratively with the Hospice Home team in achieving patient care goals Maintain comfort, privacy, and dignity of our patients Promptly reporting pertinent information to a member of the leadership team. Requirements: Current CNA listing with NC Home Health Aide Registry Must have reliable transportation Minimum of 1 year experience Must submit and pass all applicable pre-employment and annual tests and meet all training and other applicable state and federal guidelines What you can expect from us: Hiring manager will contact you within 1-3 business days of your application submission Complete 1-2 interview(s) with the hiring manager via virtual video, or in-person Upon receiving the job offer, review job description, and complete new-hire tasks and paperwork (via UKG) Complete New-Hire Orientation and clinical skills training Receive continuous training from our team as you continue to provide exceptional care to our patients Essential Functions: To perform this job successfully, an individual must be able to perform each essential duty.
