As the Support Coordinator, you will collaborate with the Program Manager and DSP Team to achieve goals of the people supported while providing person-centered care. Location: Chaska, MN Shift: (40 Hours) Monday/Tuesday/Thursday/Friday 2-10:30pm Every other Saturday/Sunday 4:30-10:30pm Wage: $18.50/HR Job Responsibilities
Meal planning and grocery shopping Budgeting and managing household bills Assisting with scheduling, coaching, and training of Direct Support Professionals Scheduling and accompanying individuals to medical appointments Coaching and collaboration with the team Provide supports tailored to the individual's need (this may include Activities of Daily Living, responding to behavioral situations, coaching through different situations, participation in home and community activities, etc.) Managing and administering medications Documenting care accurately to ensure ongoing support Maintain a safe and structured environment Requirements
1+ years of DSP or related experience 1 year of leadership experience (preferred) Must be at least 18 years old Valid MN driver's license Reliable transportation with valid insurance Ability to pass a DHS Background Check Summary of Available Benefits:
