Division/Program: Adult FSP (Outpatient/Community Based) Starting Compensation: 24.00 - 32.00 USD Per Hour Working Location: Fresno, CA Working Hours/Shift: Monday-Friday 8AM-5PM Why Join Our Team? What You Bring to SBHG: Associate degree in a social service field plus two (2) years of experience working in a mental/behavioral health field required (Internship may be substituted for a portion of the work experience) OR Bachelor's degree in a social science field plus six (6) months of experience preferred. (Internship may be substituted for a portion of the work experience) Valid California Driver's License required. How You Will Make a Difference: Provide one-on-one vocational services including resume development, job search assistance, interview preparation, and workplace skills training. Conduct employment assessments to identify client strengths, preferences, and employment goals. Establish and maintain relationships with local employers and community organizations to identify job opportunities and advocate for clients. Assist clients in navigating job applications, onboarding processes, and workplace accommodations when needed. Provide on-the-job coaching and follow-up support to help clients succeed and retain employment. Collaborate with clinical staff, case managers, and other service providers to integrate vocational goals into treatment planning. Document client progress and outcomes following agency and funding requirements. Participate in multidisciplinary team meetings, trainings, and quality improvement efforts. Incorporates the Transition to Independence Process (TIP) into services. Documents all services provided clearly and concisely within the Electronic Health Record within the timelines and framework established by Central Star.
