16 hours/pay period (guaranteed pay for 16 hours/pay period)

Responsible for providing support to patients, families, and caregivers by triaging calls and making visits to meet patient needs. Plan, coordinate, provide and document care for terminally ill patients from day of admission through day of discharge. Utilizing physicians' orders and professional skills, develop and implement a hospice plan that meets each patient's specific needs and is in compliance with federal and state regulations, reimbursement guidelines as well as the organization's policies and procedures. The foundation of hospice nursing is a complete skilled assessment, communicating with care team and patient/family, along with following the plan of care to meet the needs of the patient and the family during the terminal process. Key Responsibilities

Provide Nursing Care for Hospice Clients - Able to perform all visit types and functions, including but not limited to admissions, routine/PRN visits, discharges and deaths. Performs accurate and comprehensive assessments completed at the time of the visit, with documentation completed in the home. Assist the patient and family in creating an individualized plan of care to best meet their individual goals. Perform all patient care responsibilities considering needs specific to the patient/family's age, and cultural/spiritual practices. Demonstrates knowledge of comfort medications, and hospice standing orders and their administration indication. Utilize appropriate pain and symptom management techniques. Provide clinical education and ongoing support to the patient and family/caregiver. Actively participates in the bereavement plan for all families. Knowledgeable of current federal, state, and organization regulations concerning patient care. Active Member of the IDG - Communicate and collaborate on a continuous basis with all applicable members of the patients care team. Refer patients needs appropriately to ancillary services available. Accountability/Leadership - Practices accountability to daily work tasks, expectations and team norms. Identifies what needs to be done and proactively takes action to accomplish objectives and achieve company goals and priorities. Creates a positive culture and working environment within the organization. Inspires, motivates, encourages and guides others toward goal accomplishment. Demonstrates and fosters a sense of team spirit, pride, trust, and commitment. Education and Experience Required

Current Registered Nurse licensure in the state of Minnesota. Must obtain CPR certification within 6 months from date of hire. Must have a car with required insurance coverage and a state driver's license to travel to clients' residences. Must have adequate computer skills. Must complete and pass a state required background study. Able to communicate verbally and in writing to the extent required by the position. Able to physically perform the duties required by the position. Required to have the ability to read, write and speak English well enough to communicate with staff, clients and visitors.
