Outreach/Navigator
Job Title: Outreach/Navigator Department: Healthy Living Location: 211 East Main Street, Batavia, N.Y. 14020 Hours Per Week: 15/Hrs per week Schedule: Days & Evenings Sign-On Bonus: N/A Summary: Responsible for breast, cervical and colorectal cancer prevention and early detection education, promotion of the CSP, targeted outreach activities and patient navigation. Reports To: Cancer Services Program Coordinator Responsibilities:
Provides breast, cervical and colorectal cancer prevention and early detection education to the lay community, targeting CSP priority populations. Participates in a minimum of 6 events per quarter. Serves as liaison between CSP, CSP-eligible clients, and partner organizations. Garners at least one earned media per quarter. Makes contact with key stake holders or government decision makers in their area on a quarterly basis. Works with one provider or healthcare system for patient navigation. Obtains baseline data and works with providers to develop and implement strategies to improve screening rates of the priority population per the guidelines of the CSP grant. Attends CSP staff meetings and local and state meetings as scheduled. Completes monthly reports documenting outreach activities, earned media, contacts with key stakeholders and patient navigation progress. Age Specific Care: Provides age and culturally appropriate assessment, interpretation of clinical and laboratory data and develops and implements age appropriate interventions and plans of care including education within the parameters of his/her position responsibilities and licensure. Ensure staff considers age related concerns such as: hearing, vision, speech and level of understanding, when interacting with patients, employees and families. Required Qualifications:
Familiar with medical terminology and computer competence. Preferred Qualifications:
Must have ability to read, write and speak English. Community outreach experience preferred, as well as previous experience in public speaking. Familiar with medical terminology and computer competence Education: AS (Required) Physical Requirements: S - Sedentary Work - Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met. For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements.
