Clinical Pharmacist
The Clinical Pharmacist provides distributive and clinical pharmacy services in a professional and competent manner to assure rational and appropriate drug therapy for all patients. Responsibilities include: Pharmaceutical Management: Provide specialized pharmaceutical services to improve drug usage and therapeutic outcomes; advise providers on issues concerning drug therapy, side effects and dose management. Leadership & Relationship Development: Mentor/supervise pharmacists in training; coordinate the activities of assigned pharmacy technicians; maintain liaison relationship with medical and nursing staff. Evaluation: Conduct and evaluate medication histories, assess compliance and suggest modifications. Required qualifications include: Degree in Pharmacy from an accredited college of Pharmacy. Licensure as a Pharmacist in New York State. Preferred qualifications include: Minimum 4 years of pharmacy practice experience in a hospital setting. Strong analytical, computer, and communication skills with an excellent attention to detail. Education: BS: Pharmacy (Required) Licenses/Certifications: LPharm - Licensed Pharmacist - New York State Education Department (NYSED) Physical Requirements: Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
