The Esperanza Center provides a welcoming atmosphere, social services, health services, legal support and English as a Second Language (ESL) to immigrants in the Baltimore metropolitan area. The center's staff offers assistance with employment, English proficiency, legal services, and access to primary and urgent health care and referral services. Case Manager (Bilingual English/Spanish)

who provides intensive case management including, but not limited to, as-needed home visitation services to participants/clients. The Case Manager assesses household needs, coordinates resources based on goals, and provides advocacy, information, and referral services. The work schedule is Monday - Friday 8:30am-4:30pm business hours 37.5 hours per week- This is an onsite position only. JOB DUTIES & RESPONSIBILITIES: The Home Study Case Manager (HS CM) will conduct home visits and phone calls to assess potential risks of placement & determine whether the sponsor/caregiver & proposed environment are suitable. Provide sponsors with psychosocial education on the minor's unique needs. Identify family strengths and social support. Help families develop an individualized plan for successful integration If needed, provide ongoing case management focusing on screening and making appropriate referrals to address the minor's needs related to placement issues, safety issues, education, health, mental health, legal/immigration proceedings, substance abuse, independent living, criminal/juvenile justice and child enrichments activities. Complete and document all required reports, case notes, and contacts in the appropriate database by the expected timeframes and requirements. Collaborate with CBOs, law enforcement agencies, or government office to ensure the safety and well-being of the minor. Provides crisis intervention services as needed Participate in outreach and networking events in the community to try to leverage more resources for the families Assist in other special projects for the family reunification program as needed. Perform other duties as assigned. EDUCATION & EXPERIENCE REQUIREMENTS: Case Manager: Bachelor's degree in Human Services, Social Work, or related field Two years related experience or an equivalent combination of education and experience REQUIREMENTS & ABILITIES: Must be fluent in both Spanish and English, and be culturally competent to deal with clients from diverse Latino/Hispanic backgrounds. Excellent relational skills. Ability to remain calm in stressful situations. Excellent knowledge of case management and crisis intervention services. Excellent communication skills including active listening. Excellent verbal and written communication skills. Excellent relational and supervisory skills. Ability to act with discretion and professionalism in all situations. Exceptional planning and organizational skills. Effective conflict resolution skills. Driving (Agency and/or personal vehicle) is required for this position. Driver must be at least 21 years of age, with a minimum of 2 years of driving experience, and a valid driver's license issued by the state of residence. There can be no state-issued restrictions on the licenses that would impede driver's ability to operate the vehicle as required by the Agency, and there must not be more than three (3) points on their driving record. Basic Windows PC, web browsing (i.e. Chrome, Internet Explorer, etc.), and Microsoft Outlook skills Other Microsoft Office application knowledge, such as Word, Excel PowerPoint, Skype, and OneDrive desired. PHYSICAL REQUIREMENTS & WORK ENVIRONMENT: Sedentary work that primarily involves sitting/standing. Remaining in a stationary position, often standing, or sitting for prolonged periods. Communicating with others to exchange information. Repeating motions that may include the wrists, hands and/or fingers. Assessing the accuracy, neatness and thoroughness of the work assigned. Hearing and vision abilities within normal range (corrected) to drive safely and to observe and communicate with others. Ability to lift 20 pounds.
