MindCure, Inc. is seeking a compassionate, resourceful ECM Care Manager to join our Community Connect team. In this role, you will provide care coordination and direct services under Californias Enhanced Care Management (ECM) Program, helping clients navigate complex health and social service systems. Youll work as part of a multidisciplinary care team to promote wellness, recovery, and independence for vulnerable individuals in our community.
Key Responsibilities
Assess client needs related to physical and mental health, SUD, trauma, housing, employment, and social supports
Develop and manage Individual Care Plans (ICP) and Health Action Plans (HAP)
Provide support through in-person visits, home/community-based care, and telehealth
Link clients with relevant health providers, social service agencies, and community resources
Advocate for clients and empower them in self-advocacy
Use trauma-informed care, motivational interviewing, and harm reduction techniques
Coordinate services with a multidisciplinary team and track progress
Maintain timely, accurate records in Electronic Health Records (EHR)
Attend team meetings, supervision, and required trainings
This position involves a mix of office-based work, field visits, and virtual services. Daily tasks may include driving to client homes, attending team meetings, and completing EHR documentation. Physical activity includes sitting, standing, walking, and moderate movement throughout the day.
