is seeking a dynamic Transitional Care Nurse Practitioner to join our inpatient care management team at Palisades Medical Center. In this pivotal role, you will be a central driver of patient care, collaborating with physicians and the interdisciplinary team to enhance clinical outcomes, improve patient throughput, and ensure a seamless care journey from admission to discharge. This is a full-time, day shift position featuring four 10-hour shifts (7:30 AM - 6:00 PM) after orientation. Responsibilities: Conduct comprehensive assessments, order and interpret diagnostic tests, and develop evidence-based treatment plans for adult inpatients. Prescribe medications and provide emergency care within the scope of your clinical privileges and in collaboration with physicians. Serve as the primary clinical liaison between physicians, nursing staff, patients, and families to coordinate all aspects of patient care. Ensure clinical documentation is accurate, timely, and supports patient acuity and quality metrics. Drive patient education on medical conditions, medications, and health promotion to empower them in their care. Act as a clinical resource and mentor for nursing staff, promoting best practices and a collaborative team environment. Participate in quality improvement projects and the review of clinical policies to advance evidence-based care. Identify and help resolve ethical care issues through interdisciplinary collaboration. Requirements: Required: Master's Degree in Nursing (MSN) from an accredited program Current NJ State Advanced Practice Nurse (APN) and Registered Nurse (RN) licenses AHA Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications Excellent communication and collaboration skills Eligible for credentialing with the Medical Staff Preferred: At least 1 year of experience as a Nurse Practitioner NJ CDS, DEA, NPI, and PECOS certifications COMPENSATION: Starting Base Salary: $123,260.80 Annually HMH is committed to pay equity and transparency for our team members.
