You will provide direct clinical services to pediatric patients as part of the Collaborative Care Model (CoCM) within a primary care clinic, collaborating closely with Optum Behavioral Care (OBC). In this innovative role, you will connect with patients, primary care providers (PCPs), and psychiatrists to support the mental and physical health of an assigned patient caseload.

Woodhaven Pediatrics , Monday through Friday, enjoying a schedule with no evenings, weekends, or holidays. Licensed Master Social Worker (LMSW) Licensed Professional Counselor (LPC) Licensed Marriage and Family Therapist (LMFT)
Experience providing behavioral health services, particularly to pediatric patients Over 3 years of experience treating behavioral health conditions At least 1 year of experience in an integrated primary care setting Familiarity with evidence-based psychosocial treatments and brief interventions Eligibility for enrollment as a Medicare provider or ability to opt-out Preferred Qualifications: Experience in the Collaborative Care Model (CoCM) Strong organizational skills and exceptional follow-up capabilities Proven ability to maintain professional relationships with patients and care team members Comfortable working with patients via in-person, telephone, and video Solid grasp of technology and ability to adapt to various tools Salary is based on various factors including local labor markets, education, experience, and certifications. The annual salary for this role ranges from $58,800 to $105,000 based on full-time employment. A comprehensive benefits package is also available.
