Job Description
Purpose of Position:
Provides person-centered, strength-based assessments, evaluating barriers to access and adherence that are tailored to develop treatment/care plans focused on the individual needs and goals of the patient. The assessments will integrate health care a social support service
Responsibilities:
Develop an interdisciplinary, patient-centered care plan Ensure patients with multiple chronic conditions/ disabilities achieve the highest personal level of functioning Improve communication among providers/ care team, patients, and their caregivers Offer a range of services for high-risk patients, including psychotherapy, and social support programs Support family caregivers to reduce stress and burdens Minimum Qualifications
For Assignment to Level II 1. Valid license as an LMSW issued by the NYSED; or 2. Valid license as an LCSW issued by the NYSED.
Department Preferences
Tour 2: 9am-5pm
