Job Description
Purpose of Position:
Provide patient care services in Behavior Health. To provide patients with high risk assessments, mental health services and ensure safe discharge planning back into the community
Roles and Responsibilities:
Complete psychosocial assessments on all new admissions to determine patient's healthcare needs Formulate and initiate appropriate discharge planning options. Responsible for all documentation that will facilitate patient's discharge Facilitate and organize outpatient care with other providers Be an active part of the clinical team, arrange family meetings and provide family and individual therapy in inpatient settings Maintain statistic, and excellent chart documentation in accordance with hospital policy Observe and identify verbal and non-verbal behaviors. Communicates listens and elicit expression to understand patients' family perception of the situation. Identify the elements involved in a medical situation and their relationship to the patients Minimum Qualifications
For Assignment to Level II 1. Valid license as an LMSW issued by the NYSED; or 2. Valid license as an LCSW issued by the NYSED.
Department Preferences
Experience in dealing with mentally ill patients and knowledge of community resources
Tour 1: 12am-8am
