Job Description Job Title: LPN - FT Nights Profession: Licensed Practical Nurse Specialty: Long-Term Care Duration: 13 Weeks Shift: FT Nights Hours per Shift: 5x8 Experience: 1 Year as LPN License: NY LPN Licensure Certifications: BLS (AHA or RQI only) Must-Have: 1 Year Experience as LPN

Description: The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit.

The individual must demonstrate knowledge of the principles of growth and development across the life span.

The LPN must possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to age-specific needs.

The LPN provides direct and indirect skilled nursing care for assigned residents.

The LPN follows defined policies and procedures under the supervision of a Registered Nurse.

The LPN maintains a collaborative relationship with other nursing staff, physicians, and ancillary staff.

The LPN is expected to maintain an appropriate level of knowledge and clinical expertise through continuing education and in-service programs.

The LPN shall have the ability to provide care to geriatric residents.

Job Duties: Collect data relevant to residents' physical and psychosocial condition on an ongoing basis.

Communicate findings to the Registered Nurse and other members of the Interdisciplinary Team.

Contribute to the creation of a developmentally age-appropriate and culturally relevant plan of resident care.

Reflect nursing diagnosis and interventions to achieve desired resident outcomes while involving the resident, family, and healthcare team.

Promote a healthy and safe environment.

Recommend revisions to the plan of care after evaluating resident outcomes.

Indicate the need for referrals to ensure all aspects of the resident's care are addressed.

Collaborate with residents, families, and members of the Interdisciplinary Team within the scope of nursing practice.

Assure customer satisfaction.

Maintain current knowledge and competency in nursing practice.

Participate in the mentoring and orientation of staff.

Consider factors related to safety, cost, and impact when planning the delivery of care.

Under the direction of the Registered Nurse, provide leadership in the professional practice setting.

Demonstrate knowledge and competency in nursing practice.

Occupational exposure to blood borne pathogens is reasonably anticipated.

Required tasks involve potential contact with blood, body fluids, or tissues.
