Implement dynamic health education programming in both individual and group settings utilizing innovative teaching methods. Employ agile navigation techniques and motivational interviewing to guide patients in setting and achieving health goals. Provide data-driven information, referrals, and assist with seamless service linkage. Conduct state-of-the-art point-of-care testing for HIV and other health conditions. Contribute to the development of breakthrough health education programs. Collaborate within an interdisciplinary care team to enhance patient outcomes. Perform brief, tech-enhanced assessments of patient needs, readiness for behavior change, and track progress over time. Establish and maintain strategic referral processes and community networks. Promote timely access to care through digital solutions and teach self-management skills. Document patient services using electronic health records with precision and accuracy. Manage patient schedules and achieve performance metrics using data analytics. Minimum Qualifications
High School Diploma or equivalent; Bachelor's Degree in a related field preferred. 3 - 5 years of relevant experience in a healthcare or community services environment. Minimum of 1 year of relevant experience if holding a Bachelor's Degree. Proficient in performing point-of-care testing with a tech-savvy approach. Expertise in HIV care and prevention strategies. Core competencies of HIV Prevention within 3 months of hire. Phlebotomy training preferred. Bilingual English/Spanish highly preferred to enhance communication and outreach. Proficient in Microsoft Word, Excel, and other advanced computer programs. About Legacy Community Health
