Position Highlights:
- Full-time position (5 days/week) providing primary care in 2 skilled nursing facilities and 1 assisted living facility

Job Duties:
- Provide primary care rounding in SNF and ALF settings
- Diagnose and manage acute and chronic medical conditions
- Develop and implement individualized care plans
- Conduct clinical case reviews and share best practices
- Foster strong relationships with patients, families, and facility staff

- Estimated annual compensation: $210,579 $271,579 (no cap on productivity income)

Candidate Requirements:
- Board-certified or board-eligible physician
- Experience in post-acute care preferred but not required
- Active and unrestricted Delaware medical license (or in process)
- DEA license with prescriptive authority
