Summary/Objective
Under the direct supervision of the Director of Case Management, the Master's Social Worker (MSW) plays a vital role in assisting patients with complex psychosocial needs. This role involves determining eligibility for social programs, identifying funding sources, and connecting patients to community resources. The MSW provides crisis intervention for patients and families and collaborates with interdisciplinary team members to develop and implement comprehensive discharge plans for high-risk patient populations.
Essential Job Duties

Evaluate patients' and families' psychosocial risk factors by assessing prior functioning levels, the adequacy of support systems, reactions to illness, and coping abilities. Provide emotional, social, and financial support to patients and families facing the consequences of illness or disability. Mobilize family and community resources to address identified needs effectively. Intervene in cases of child abuse/neglect, domestic violence, elder abuse, institutional abuse, and sexual assault. Collaborate with the care team to develop and execute discharge plans for complex cases, ensuring timely transitions and appropriate post-discharge care coordination. Assist Case Managers, Bachelor's Social Workers, and Patient Navigators in coordinating with significant or intensive community resources to achieve desired treatment outcomes. Educate patients, families, and physicians on post-acute care options and provide guidance on making informed choices.
Required Education and Experience
Master's degree in Social Work
1-3 years of related experience.
Required License/Certifications
Licensed Social Worker in State of Texas
