Summary
Job Description
The HCC Coding Coordinator assists with processing Annual Wellness Visit forms and documentation, preparing HCC/RAF reports, compiling training materials and supporting providers/clinics and the HCC Coding team as needed. A major focus is coordinating the timely and accurate completion of Annual Wellness Visits (AWV) for Medicare and Covered CA members. Candidate is expected to be an active participant in continuous quality improvement processes and workgroups and maintain a strong partnership with HCC coders and other Quality Management teammates.
Duties And Responsibilities
- Prepare and send forms and reports to clinics including, but not limited to, Annual Wellness Visit, Eligibility, Third-Party Vendors report, and PCP Visits Update outreach lists for clinics and third-party vendors
- Review and abstract medical records for completeness and enter data accurately into Cozeva and other platforms
- Communicate documentation deficiencies to providers
- Apply knowledge of medical coding guidelines and regulations to assist providers and clinic staff
- Review and submit completed forms and supporting documentation to health plans
- Download, organize and post training materials, reports and reference documents for use by Coding Team and providers/clinics
- Create forms, correspondence, memos and other materials as directed
- Provide calendaring and other administrative support to Coding team as directed
- Answer and direct Coding team calls, emails and faxes as directed
- Other duties in support of Coding Specialists/Analysts as directed
- May require driving to meetings and Southern California clinics
- Ability to be organized and multitask to complete projects timely and accurately
- Strong written and verbal communication skills, including phone etiquette
- Use independent judgement and sensitivity to communicate with providers and clinic staff
- Maintain a professional and supportive working relationship with clinic staff, health plan staff, administration, and physicians
- Participate in video conferences, phone calls and in-person meetings as requested to establish and maintain interdepartmental and external partner communication and cooperation
- Understand and adhere to HIPAA requirements and all Company policies
- Proficiency with Microsoft Office Programs, especially Word and Excel
- Knowledge of Sharepoint a plus
- Current CA Drivers license with current auto insurance
