Summary

SUMMARY:

A Community Health Officer is a licensed physician or public health professional who serves as a bridge between clinical care, public health policy, community needs and FQHC development. They may work within public health departments, county health systems, or FQHCs, focusing on improving health outcomes in underserved or medically vulnerable populations. These roles are critical in addressing health disparities, managing public health programs, and ensuring compliance with federal and state regulations, such as those governing FQHCs.

DUTIES & ESSENTIAL JOB FUNCTIONS: NOTE: Following are the duties performed by employees in this classification. However, employees may perform other related duties at an equivalent level. Not all duties listed are necessarily performed by each individual in the classification.

Public Health Leadership and Policy Enforcement: Oversee and enforce public health regulations, including communicable disease control, immunizations, and emergency preparedness, in collaboration with public health authorities.

Public Health Leadership and Policy Enforcement: Develop and implement public health programs focused on disease prevention, health promotion, and community well-being.

Public Health Leadership and Policy Enforcement: Advise local officials and stakeholders on public health matters, disseminating information and promoting health education.

Coordination with County Health Resources: Administer programs under the direction of county health departments or boards, including managing budgets, staff, and contracts.

Coordination with County Health Resources: Collaborate with county health departments to allocate resources effectively, such as integrating services with local hospitals, clinics, or social services.

Coordination with County Health Resources: Facilitate access to comprehensive health and social services by establishing agreements with other providers to avoid duplication and maximize resources.

Coordination with County Health Resources: Acts as main contact for Alameda County Public Health Officer.

FQHC Operations and Licensure Compliance: Ensure compliance with Health Resources and Services Administration (HRSA) requirements for FQHCs, such as providing care on a sliding fee scale, serving underserved populations, and maintaining a patient-majority governing board (at least 51% patient representation).

FQHC Operations and Licensure Compliance: Support the transition of health centers to FQHC status, ensuring adherence to Section 330 of the Public Health Service Act, which governs funding and operations.

FQHC Operations and Licensure Compliance: Manage reporting requirements to HRSA, including Uniform Data System (UDS) outcomes, to demonstrate quality care delivery.

FQHC Operations and Licensure Compliance: Bridges FQHC regulations with AHS strategy related to the growth and development of Community Clinics.

Community Engagement and Health Equity: Engage with communities to raise awareness about public health issues, preventive measures, and available healthcare resources.

Community Engagement and Health Equity: Address health disparities by advocating for equitable access to services, particularly for low-income, uninsured, or minority populations.

Community Engagement and Health Equity: Work with FQHC boards to ensure services are responsive to the cultural and health needs of the community

MINIMUM QUALIFICATIONS:

Required Education: Typically requires a medical degree (MD or DO) with a valid license to practice medicine in the state of employment (e.g., issued by the state's Medical Board). Some roles may accept equivalent experience, such as a Master's in Public Health (MPH) Doctor of Public Health, DrPH or significant public health experience, though physician-led roles often require medical licensure. Additional certifications, such as a National Provider Identifier (NPI) and Drug Enforcement Administration (DEA) registration, may be required.

Preferred Education: Some roles may accept equivalent experience, such as a Master's in Public Health (MPH) Doctor of Public Health, DrPH or significant public health experience, though physician-led roles often require medical licensure. Additional certifications, such as a National Provider Identifier (NPI) and Drug Enforcement Administration (DEA) registration, may be required.

Required Experience: At least 2-5 years of experience in public health, clinical practice, or a related field, often with a focus on underserved populations. Familiarity with public health principles, epidemiology, and relevant regulations at local, state, and federal levels.

Pay Range: $107.89 - $179.81 / Hour

The pay range for this position reflects the base pay scale for the role at Alameda Health System. Final compensation will be determined based on several factors, including but not limited to a candidate's experience, education, skills, licensure and certifications, departmental equity, applicable collective bargaining agreements, and the operational needs of the organization.
