Case Management Social Worker - Social Service - 20 Hours - Days Eligible for Part-Time Benefits Shift: Days 7:30 am to 4:30 pm, occasional weekend and holidays required. General Summary
Coordinates the overall interdisciplinary plan of care for patient, from admission to discharge. Responsibilities
Creates plan for care across the continuum, integrating patient/family preferences and values. Serves as a resource to assess, evaluate, recommend and treat legal, neglect, substance abuse, crisis, psychosocial and various adjustment needs. Advocates for resources and removal of barriers. Maintains ongoing dialog with supervisor and care transition team members to ensure effective implementation and reevaluation of health plan. Acts as a resource for adequate medical record documentation, complex level of care recommendations, and services as they relate to diagnoses, and treatment options for post-discharge care. Education/Experience Required
Bachelor's degree required. Education/Experience Preferred
Master's degree preferred. Acute Care Social Work experience preferred. Cerner/Allscripts/OBRA/Outlook/Google experience preferred. Certification/Licences Required
One or more of the following required: Counselor credentialed from the Michigan Board of Counseling obtained prior to hire date or job transfer date. Social Worker credentialed from the Michigan Board of Social Work obtained prior to hire date or job transfer date. Any level accepted. Certification/Licences Preferred
Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM-SW or CCM preferred.
